Florida uses the federal income and savings limits, and you apply on a paper form that you mail or fax to the Department of Children and Families.

How to apply in Florida

By phone
850-300-4323
In person
Florida asks you to mail or fax the paper form to a local DCF Customer Service Center; the DCF Family Resource Center locator lists centers near you.
By mail
Print the Medicaid/Medicare Buy-In Application from the DCF forms site, fill it in, and mail or fax it to a local DCF Customer Service Center. Florida says this paper form is the only way to apply for Medicare Buy-In alone.

Good to know

  • Florida's chart lists each limit after the first $20 of monthly income is set aside, so $1,330 here lines up with the federal $1,350 for QMB, and the other limits line up the same way.
  • Florida's chart also lists higher limits for larger families, up to 10 people (for example, $2,277 a month for QMB in a family of three).
